A local healthcare provider in Coventry is seeking an Advanced Clinical Practitioner to provide expert-level clinical care. Working hours: Full time, 40 hours per week Monday - Friday

ACP experience in primary care is essential for this role. Applicants must be able to evidence this in their application.

Main duties of the job

The Advanced Clinical Practitioner will provide autonomous, expert-level clinical care, managing patients across a range of acute and chronic conditions. The ACP plays a key role in reducing unnecessary GP referrals, ensuring patients receive timely and effective care, and developing long-term relationships with patients to support optimal health outcomes.

As an ACP, you will make independent clinical decisions, interpret test results, prescribe medications, and refer patients to specialists when appropriate.

Job responsibilities

Assess, diagnose, and manage patients with acute and chronic conditions.

Order, interpret, and act on investigations (blood tests, imaging, ECGs, spirometry).

Prescribe and review medications independently.

Perform minor procedures (wound care, sutures/staples, joint injections if trained).

Develop and implement individualised care and treatment plans.

Manage long-term condition clinics (diabetes, hypertension, COPD).

Provide health promotion, disease prevention, and lifestyle advice.

Triage patients and decide on escalation or referral to GPs or specialists.

Provide patient education, self-management support, and follow-up care.

Contribute to service planning and improvement initiatives.

Support clinical audits and quality improvement projects.

Support practice in implementing evidence-based guidelines and protocols (NICE, local policies).

Provide clinical leadership to junior staff, nurses, or other allied health professionals.

Use data and insights to improve patient outcomes and service efficiency.

Maintain accurate and detailed patient records.

Ensure compliance with data protection, clinical governance, and safeguarding policies.

Participate in revalidation, CPD, and professional development activities.

Contribute to multidisciplinary team meetings and case discussions.

Qualifications
  • Registered Nurse, Paramedic, or relevant healthcare professional with NMC/HCPC registration.
  • Meets the standards for Advanced Clinical Practice (ACP) at advanced level.
  • Experience working in primary care as an ACP.
  • Meets professional revalidation requirements.
  • Masters degree in Advanced Clinical Practice.
  • Experience in prescribing and conducting medication reviews.
