Medicines Management& Pharmacy Services at Leeds Teaching Hospitals are recruiting to avacancy within our specialist renal pharmacy team.

LTHTRenal Service provides high quality care for the population of Leeds and thesurrounding areas as one of the largest regional specialised services forpre-dialysis, haemodialysis and peritoneal dialysis. The Renal Department isalso the regional centre for kidney transplantation and performs around 200transplants per year. Following repatriation of transplant immunosuppressantmedication our pharmacy team prescribe essential transplant medication andare fully responsible for the prescription management process. We aredeveloping pharmacist-led medicines optimisation outpatient clinics to helpthese patients get the best from their medicines.

Main duties of the job

Workingclosely with the multidisciplinary team, our specialist renal pharmacists andtechnicians are integral to the safe and effective use of medicines, you willlead on significant developments and innovative strategies to improve themedicines service available to all our patients and other users.

Youwill attend consultant-led ward rounds, provide pharmacy expertise intoclinical and financial governance and strategic development of renalservices. You will facilitate the education and training of pharmacists andother healthcare professionals in the field of renal and transplant medicinesand have opportunities to develop your clinical, leadership, managerial andquality improvement skills.

Ourspecialist renal pharmacists and technicians are integral to the safe andeffective use of medicines. You will lead on significant developments andinnovative strategies to improve the medicines service available to all ourpatients and other users.

JOBPURPOSE

Tolead, deliver, develop and evaluate highly specialised clinical pharmacyservices to Renal Services.

JOBDIMENSIONS

Leads the highly specialisedclinical pharmacy service to patients and the multidisciplinary team within agiven clinical speciality. Ensures medicines are used appropriately, safely,and cost-effectively in accordance with Trust policy, standard operatingprocedures and medicine legislation. The post holder is expected to determinehow the outcomes are best achieved in specific circumstances.

Manage other pharmacists providingspecialised clinical pharmacy service within own clinical specialitydepending on size of area.

Works directly and regularly withconsultants, senior nursing staff and managers within the clinicalspeciality. Monitors and interpretsexpenditure on medicines within own clinical speciality. Provides theDirectorate with regular reports to assist in the safe use of medicines, themanagement of the drug budget and clinical governance issues. The report alsoincludes risk management such as the recording of significant clinicalinterventions and near misses.

Supervise, along with other membersof their multidisciplinary team, the development and implementation oftreatment protocols, and guidelines for use of drugs within the clinicalarea.

Responsible for trainingpharmacists, and other health care professionals, on aspects of drug usewithin the clinical area.

Provides minimum of at least one 4hour session of service cover in the dispensary or aseptic dispensary eachweek. Participates in evening, weekend and bank holiday rota and whereappropriate, out of hours services.

To act as the ResponsiblePharmacist in line with local procedures

Person Specification

Experience

• Substantial post registration experience as a practising clinical pharmacist, including experience of renal medicine
• Ability to demonstrate foundation level competencies in all 6 domains of the Advanced to Consultant Level Framework (ACLF)
• Experience of dealing with highly complex clinical issues which require the pharmacist to analyse information from a range of sources
• Experience of dealing with a wide variety and grade of hospital staff (including senior medical and managerial staff) on a regular and ongoing basis
• Understanding and appreciation of local and national pharmaceutical and clinical strategies

Qualifications

• Masters qualification in Pharmacy (4 years) or equivalent qualification plus one year pre-registration experience
• Registrant of the GPhC
• Postgraduate diploma in clinical pharmacy or equivalent experience or qualification
• Non-medical prescriber
