Adults Social Worker - Independence & Access Team

Location: Haringey
Working Pattern: Hybrid (minimum 1 day per week in the office + regular local assessment visits)
Duty Commitment: 1 week in the office every 4 weeks
Contract: Full-time
Requirements: Qualified Social Worker + SWE Registration + relevant Adult Social Care experience

What you’ll do

• Carry out local assessment visits to understand needs, build trust, and co‑produce clear support plans.
• Work collaboratively with individuals, families, carers, and multidisciplinary partners to promote independence and wellbeing.
• Take part in a 4‑week duty rota, spending that week office‑based to support urgent cases, triage enquiries, and provide a responsive service to residents.
• Support adults with a wide spectrum of needs, including physical disabilities, long‑term conditions, learning disabilities, mental health challenges, and safeguarding concerns.
• Use your professional judgement to complete assessments, reviews, risk plans, and safeguarding work in line with Care Act principles.
