Adults Social Worker within Community in Bristol UK earning £32 Hourly Ongoing. Full time. This is an exciting opportunity for a skilled Adults Social Worker to join our community team in the vibrant city of Bristol. Responsibilities
  • Conduct comprehensive assessments under the Care Act, facilitate carer assessments, and support adults with care and support needs through insightful planning and thoughtful reviews.
  • Lead safeguarding enquiries under Section 42 and undertake meaningful assessments in line with the Mental Capacity Act.
  • Manage a diverse and autonomous caseload, typically consisting of 16–20 individuals, with a focus on prioritising urgent visits and strategic risk planning.
  • Empower individuals through a strengths-based, person-centred approach that promotes their choice and control over care decisions.
  • Collaborate effectively with health providers, housing organisations, voluntary groups, and multi-disciplinary colleagues to ensure holistic care.
  • Comply with internal key performance indicators, maintaining precise case records promptly, and adhering to statutory reporting obligations aligned with legal and organisational standards.
  • Engage in duty rota responsibilities, contribute to multi-agency meetings, and facilitate mental health discharge meetings.
