Adult Social Worker – Hospital Team – Southampton

Pay rate £30 per hour

Pertemps are recruiting for Adult Social Workers in Southampton.

We are looking for a dedicated and compassionate Hospital Social Worker to join our integrated discharge team based at Southampton General Hospital. This role is central to supporting safe, timely discharges and promoting independence for adults with complex needs.

You will be based in the hospital and will also carry out community visits to review hospital discharges. Face‑to‑face assessments are conducted on the wards, and transport is required for community‑based work.

There is plenty of hybrid working available.
