Job description

We have an opportunity for an experienced Heart Failure nurse to lead our Acute Heart Failure Team .We are looking for someone who is driven, enthusiastic and innovative with excellent experience and expertise in the care and management of patients with Heart Failure. You will work with our existing established Integrated Heart Failure Team within the hospital.

In addition to managing a caseload of inpatient heart failure patients, the post holder will be involved in developing innovative heart failure pathways to transform the way Heart Failure patients receive care. We are continuously reviewing and updating our ways of working, aiming to provide outstanding collaborative care.

You will be an experienced and committed nurse who is motivated and energized to change the way services are delivered and improve patient outcomes. We are looking for someone who has had at least Minimum of 2 years' experience in managing patients with coronary heart disease plus a minimum of 1 year experience of managing patients with heart failure.

We will offer ongoing supportive supervision in practice. The opportunity to undertake further study at advanced practice level is available. This role requires you to have excellent communication skills and be confident to work under your own initiative.

Main duties of the job

Heart Failure Clinical Nurse Practitioner

The aim of this post is to optimise the management and delivery of care to people with diagnosed heart failure by providing effective treatment and improving patients' quality of life through high quality, holistic, evidence-based care. The post holder will manage their own caseload, providing personalised care plans, initiating medication and lifestyle changes through education and support to patients, carers and other healthcare professionals.

You must hold a Teaching Qualification ENB 998 or equivalent, hold a relevant first level degree and a specialist qualification or other relevant post graduate qualification in Heart Failure.

You must have advanced clinical knowledge and skills, be willing to participate in research and audit projects and will also be expected to work with a certain amount of autonomy. In return you will be offered excellent commitment to both your personal and professional development.

Qualifications
  • Registered Nurse/Professional and hold active registration with relevant Professional body
  • Degree level qualification relevant to role
  • Post Graduate qualification at Masters level relevant to role to demonstrate specialist clinical and leadership skills
  • Evidence of mandatory and statutory essential training skills, for example, Basic Life Support, manual handling, infection control, fire etc
  • Managerial or Leadership qualification
