18+ Leaving Care Personal Advisor plays a critical role in supporting young adults as they transition from care to independent living. You will work closely with your clients, providing guidance and advice on a wide range of issues, including education, employment, housing, and personal well-being. Engaging with external agencies and resources, you will help young adults navigate various challenges they may face when leaving care. Your empathetic and supportive approach will empower your clients to make informed decisions about their future, fostering their confidence and resilience. Additionally, you will maintain accurate records and ensure compliance with relevant policies and statutory requirements, all while advocating for their rights and needs. Responsibilities
  • Provide one-on-one guidance and support to young adults transitioning from care.
  • Assist clients in developing life skills necessary for independent living, including budgeting, cooking, and job readiness.
  • Help clients navigate educational opportunities, including training and further education applications.
  • Support young adults in securing stable housing and understanding their rights as tenants.
  • Collaborate with various agencies and services to coordinate support for clients, including mental health services and employment resources.
  • Ensure all case notes and documentation are maintained accurately and in accordance with data protection guidelines.
  • Advocate for the needs and rights of young adults in meetings, and when working with external organizations.
Requirements
  • A minimum of a diploma or degree in social work, youth work, or a related field.
  • Experience working with young people, particularly those from vulnerable backgrounds.
  • Strong understanding of the challenges faced by young adults leaving care.
  • Excellent communication skills, both verbal and written.
