Reporting to the Manager of Morningstar Centre, the Prevention Support Worker provides support throughout clients' recovery journey, including admission, care, and discharge. The role involves assisting through stages of change, supporting effective care transition, discharge planning, and facility functioning via group work and cross-program collaboration.
