We are looking for a full-time contract Transitional Housing Support Worker to join our Outreach team.

Position

The Transitional Housing Support Worker will provide services to unhoused individuals in Caledon, Malton and Port Credit who are homeless or at risk of homelessness, assisting with mental health and/or substance use and supporting successful transition from street to housing or navigating systems to help maintain housing.

Note: This posting is for an active vacancy. Status: Full-time contract until end of March 2026, 37.5 hours per week.

What You Will Do
  • Build client capacity, empowerment and self-reliance
  • Work collaboratively with a multidisciplinary team to move clients through a continuum of services to decrease wait times
  • Develop and initiate relationships with prospective clients based on trust, respect, and honesty
  • Collaborate with the Peel Outreach Support Landlord Engagement Specialist to support housing transitions and stability
  • Support transitions into housing using a Critical Time Intervention Approach; set short and long-term goals, create and document support plans, and make referrals as appropriate
  • Complete initial intake and discharge assessments and other required documentation
  • Connect individuals with housing options, including accompanying them to view options and being present on move-in day
  • Conduct Case Conferencing with the support system of involved persons
  • Maintain professional relationships with referral sources and service providers

What You Need to Join Our Team
  • Degree or diploma in Social Services/Social Work; a BSW is preferred
  • Minimum of 3 years of experience with the homeless in an outreach setting
  • Minimum of 3 years of case management experience
  • Diversity and cultural competence; understanding of oppression and social determinants of health
  • Experience with assessments and risk assessments
  • Familiarity with CMHA Peel programs, partnering agencies, and outside referral sources
  • Ability to work in a fast-paced environment and adapt to changing healthcare landscape
  • Knowledge of mental health, addictions, stigma reduction, and health promotion principles; knowledge of concurrent disorder is an asset
  • Strong interpersonal skills; ability to communicate across diverse cultures
  • Commitment to anti-racism, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework
  • Willingness to understand diversity elements such as gender identity, race, religion, sexual orientation, disability, immigration and precarious status, family status, etc.
  • Fluency in a second language; ASL is considered an asset
  • Valid Canadian driver’s license and access to a vehicle in good repair
  • A satisfactory Vulnerable Sector Screening (Police Check)
