Job Summary

The Service Navigator, Addictions is responsible for providing navigation support to clients with substance use issues in the Sunnybrook Emergency Department or an inpatient unit. The role will support staff and patients by creating connections between Sunnybrook’s Emergency Department, inpatient teams, the Rapid Access to Addictions Medicine Clinic (RAAM) and the community. The navigator works closely with the Peer Service Navigator to help patients access services needed to address substance use challenges and improve their quality of life. The navigator works in partnership with clients to develop a recovery plan and assists with accessing community resources, linking clients to appropriate services and developing a safe plan. The role also documents interactions with clients and other parties as required. This position actively engages with inter‑professional colleagues inside the hospital and in the community to better meet the needs of people with substance use disorder.

What You Will Do
  • Assist clients in developing and implementing a care plan tailored to their needs and issues to improve quality of life.
  • Act as a liaison and advocate for clients among family, caregivers, medical and other interdisciplinary providers.
  • Provide supportive counseling or referral and linkages to health care, programs or service providers.
  • Provide crisis management/intervention and assist in developing crisis plans for clients.
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
  • Maintain strict confidentiality and follow all required policies and procedures.
  • Consistently interact with clients, staff, family members, community agencies and partners in a professional manner that reflects favorably on the organization.
  • Attend and actively participate in all required training.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners.
  • Participate in partnership meetings as required.
  • Participate in weekly team meetings, individual supervision and regular agency administration meetings.
  • Takes responsibility for identifying areas for professional growth and training, in conjunction with the Manager or Director.
  • Participate in ad‑hoc committees within the agency, providing input to improve services.
  • Represent the Network at external workshops, inter‑agency initiatives, staff training and other community-based activities.
  • Complete other duties from time to time, as assigned by Program Director/Manager.
What You Bring
  • Completion of post‑secondary education in social services/human services or equivalent.
  • Minimum 3 years of experience supporting adults with mental health and/or substance use issues.
  • Understanding of harm reduction, psychosocial rehabilitation principles, therapeutic relationship, and ability to agree with clients on goals/priorities.
  • Crisis intervention skills with well‑developed problem‑solving and interpersonal skills.
  • Experience in group counseling related to mental health, addictions and related issues.
  • Excellent management of privacy and confidentiality of client information according to agency policy.
  • Comprehensive knowledge of mental health and housing services and networks in Toronto.
  • Accurate record‑keeping and reporting of time sheets, expense claims and other required information, in accordance with agency guidelines.
  • Good oral and written communication skills and computer literacy, including MS Word and Excel.
  • Ability to work effectively in a team environment.
  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness.
  • Understanding of cultural competency and anti‑oppression framework.
  • Proven experience with anti‑racist and DEI practices.
  • Ability to communicate in a different language; an asset.
  • A valid Ontario driver’s license.

  • May be required to be on call as an emergency contact after hours. Hours of work may include, on occasion, some early evenings.
