The Lung Health Foundation is based in Toronto and operates in a hybrid model in which staff are required to work at the office a minimum of two days each week with occasional travel within Ontario and across Canada as required.

Salary range : $82000 - $92000

POSITION OVERVIEW

The Senior Manager Person-Centered Care and Programs plays a key leadership role in advancing the Lung Health Foundation's commitment to designing and delivering high-impact evidence-informed and equitable programming. With a particular focus on the COPD and asthma portfolios this role is central to ensuring that LHF’s work reflects the real needs voices and lived experiences of people living with lung disease across Canada.

Working in close partnership with the Director Person-Centered Care and Programs and in collaboration with internal teams and external stakeholders the Senior Manager helps shape and guide the planning implementation and evaluation of person-centered initiatives. These programs aim to improve quality of life self-management and access to care for patients and caregivers while also supporting healthcare system transformation through education resources and innovation.

This role contributes significantly to the integration of evidence evaluation and equity across LHF’s work. The Senior Manager will lead or contribute to the development of program logic models evaluation frameworks outcome measurement strategies and knowledge translation tools ensuring that all programs are measurable meaningful and aligned with best practices in public health chronic disease management and health equity.

In addition to technical expertise the Senior Manager provides day-to-day leadership and support to a growing team offering coaching mentorship and performance oversight that reflects LHF’s values of respect collaboration and inclusion. They will foster a team culture that is both high-performing and psychologically safe where learning diversity of perspective and shared accountability are prioritized.

This position is ideally suited for someone who brings strong capabilities in program evaluation quantitative and mixed-methods analysis stakeholder engagement and project coordination and who is deeply committed to the principles of person-centered care health equity and anti-oppression. The successful candidate will also bring a thoughtful curious and compassionate approach to leadership and a desire to make a tangible impact on the lives of people affected by asthma and COPD.

RESPONSIBILITIES
Program Development & Strategic Direction
  • Support the planning execution and improvement of person-centered programs in asthma and COPD in collaboration with internal and external partners.
  • Contribute to identifying needs gaps and opportunities across LHFs program portfolio.
  • Help ensure programs are co-designed with community input and reflect current evidence and lived experience.
  • Translate evaluation findings and user feedback into insights to support continuous improvement and strategic growth.
Evaluation Research & Data Analysis
  • Contribute to the design and implementation of program evaluation frameworks and indicators.
  • Lead or support quantitative and mixed-methods data analysis to assess program outcomes equity of access and reach.
  • Collaborate on the development of logic models reports and dashboards to communicate progress and impact.
  • Conduct literature reviews environmental scans and needs assessments to support evidence-informed planning.
People and Team Support
  • Help guide and support the work of direct reports contributing to goal setting project planning and professional development.
  • Foster a collaborative and respectful team environment aligned with LHFs values.
  • Provide coaching mentorship and regular feedback to team members under the direction of the Director.
  • Promote a culture of learning accountability and inclusion across the team.
Stakeholder Engagement & Knowledge Exchange
  • Engage with healthcare providers community organizations and individuals living with lung disease to inform programming.
  • Represent LHF in external networks and working groups contributing to thought leadership and knowledge exchange.
  • Help facilitate co-design and feedback processes with patients and caregivers to ensure programs remain responsive and relevant.
Cross-Functional & Strategic Collaboration
  • Collaborate across departments (e.g. fundraising communications policy) to support the alignment of patient programming with broader organizational goals.
  • Assist in the preparation of materials for strategic planning internal decision-making and reporting to the Executive Team or Board.
  • Support grant writing and funding proposals by contributing data narratives and budgets as needed.
Diversity Equity and Inclusion (DEI) Skills
  • Apply a health equity lens to all aspects of program design and evaluation.
  • Ensure culturally safe inclusive and accessible programs for equity-deserving populations.
  • Champion principles of co-design and person-centeredness in collaboration with individuals affected by asthma and COPD.
  • Incorporate anti-oppression frameworks and the social determinants of health into program planning.
Lung Health Foundation Champion
  • Contribute to additional programs or initiatives based on emerging priorities.
  • Help grow and maintain LHF’s network of clinical and community partners.
  • Support cross-functional duties aligned with organizational goals.
  • Assist in external communications and advocacy efforts including media engagement in collaboration with the Marketing and Communications team.
QUALIFICATIONS
  • A degree in public health health sciences life sciences or a related field such as a Bachelor of Arts is required; graduate-level training is considered an asset.
  • It is expected that the Senior Manager will possess a minimum of six years of professional experience with respect to :
  • Program evaluation and measurement
  • Quantitative data analysis and reporting
  • Health equity or chronic disease programs (ideally COPD or asthma)
  • Community or patient engagement
  • Experience working in respiratory health primary care or health systems is a strong asset.
  • Strong understanding of evidence-based practice quality standards and patient-centered care.
  • Experience conducting and interpreting quantitative data analyses to inform decision-making program planning and evaluation.
  • Fluency in French is considered an asset.
