About Us

St. Joseph's Hospice aims to create a community that honours people as they journey to life's end and provide a haven where the palliative, the caregiver, and the bereaved experience compassion, hope, and healing. Our ten resident suites offer warm and inviting spaces where friends and family can gather together and spend meaningful time in the last hours, days, or weeks of their loved ones' lives. Hospice provides wrap-around care for the palliative individual and their family, supporting physical, emotional, and spiritual needs.

The compassionate, patient-centred care that Hospice provides is made possible by a multidisciplinary team of nurses, personal support workers, counselors, spiritual care providers, housekeeping staff, and a chef, supported by a group of loyal and empathetic volunteers.

About the Role

In this part-time permanent role, you will be responsible for providing individualized hospice and palliative care to promote comfort and quality of life for residents in your care. Responsibilities include pain and symptom management, psychosocial care for the resident and family, expert assessment and care planning, and working in partnership with an interdisciplinary team that includes a strong volunteer component. The shifts are 12 hours which includes days, nights, week-ends and holidays in a waterfall schedule.

The successful candidate will be eligible to join the Hospice Benefits Plan following 3 months of employment and our Pension Plan, HOOPP (Healthcare of Ontario Pension Plan) from first day of employment.

What you will do in this role
  • Assist in all aspects of resident care in accordance with the Regulated Health Professionals Act and the College of Nurses of Ontario Standards
  • Utilize the nursing process to provide Palliative Care to the residents and support for the family
  • Collaborate with the RN and the Director, Resident Care to dissolve any resident and family issues
  • Work with the PSW to provide personal care for all residents, as well as assist with preparing meals, light housekeeping and laundry as required
  • Perform pain and symptom assessments on an ongoing basis and report to the Registered Nurse
  • Follow the plan of care and report any resident care requirements to the Team Leader
  • Consult with the resident, family members, and / or the substitute decision maker regarding needs, wishes and preferences for care
  • Assist in the implementation and coordination of medical interventions as ordered by the attending physician
  • Provide leadership to PSWs and Volunteers
  • Document accurately and in a timely manner
  • Delegate and / or teach skills and techniques to other staff and / or resident family as appropriate and in accordance with RHPA
  • Review, evaluate and / or revise the Resident Care Plan daily.
Qualifications & Requirements
  • Practical Nurse graduate of a College or University Program
  • Current registration and in good standing with the College of Nurses of Ontario
  • Demonstrated experience working in hospice palliative care, with expertise in pain and symptom management
  • A minimum of 1 year's palliative care experience is preferred
  • Experience in geriatrics and / or care for those with chronic life-limiting illness is an asset
  • Excellent communication and interpersonal skills
  • Ability to read and comprehend simple instructions, short correspondence, and memos
  • Ability to effectively present information in one-on-one and small group settings
  • Fundamentals of Hospice Palliative Care, CAPCE, and LEAP Certificate of Completion or commitment to obtain within 2 years of start date
  • Vulnerable sector check required
