REGISTERED PRACTICAL NURSE

The Registered Practical Nurse reports to the Director of Care and is expected to coordinate and oversee all aspects of resident-focused delivery of care. Provide direct supervision, leadership and education to other members of the healthcare team. Acts as a resource advocate for residents, families, and staff. Promotes the values, goals, and philosophy of the Home.

Responsibilities:

  1. Development and modification of the plans of care in response to residents’ individualized health care needs and change in health care status
  2. Liaises with other health care personnel, agencies and families
  3. Maintains good relationships with Medical/Support/Home staff, residents, families and general public
  4. Supervises all activities on the unit, takes charge as required
  5. Monitors performance and completes performance evaluation for the PSW’s and RPN’s
  6. Directs, assigns and evaluates the work of the PSW’s
  7. May be in charge of Home as required. Utilizes leadership skills to identify problems and problem-solving on the unit
  8. Monitors that care is delivered is consistent with the resident Bill of Rights.
  9. Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
  10. Required to participate in the improvement of the indicators and achieve satisfactory results.
  11. Orders and ensure nursing supplies and equipment are properly and efficiently used
  12. Actively listens to residents and family concerns, take appropriate actions to resolve the issue in accordance with policies of the Home
  13. Conducts unit rounds at the beginning and end of shift.
  14. Processes Physician orders and administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions.
  15. Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home
  16. Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) or POA, and interdisciplinary team
  17. Completes health care assessments (RAI-MDS and other assessments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
  18. Demonstrates understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home.
  19. Other duties as assigned and or indicated in Job Task Inventory

Qualifications:

The qualifications needed to join our family are as follows:

  1. Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  2. Basic CPR
  3. Experience in long-term care setting an asset
  4. Ability to work with and relate to interdisciplinary team members
  5. Well-developed written and oral communication skills
  6. Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired.
  7. To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction.
  8. To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  9. Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
  10. As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
  11. Two supervisory references required
