This Nursing position is part of the Community Commitment Program for Nurses (CCPN) which provides a $25,000 incentive to eligible nurses in exchange for a 24-month commitment to an eligible employer. What is Pay Per Visit?
- A competitive rate that is paid per client visit, rather than care code or care time
- For longer visits or extra visits (excess of 1hr 59 min), you will be paid an additional visit
- This model allows you to work ahead in your day but also take the time you need, as you see fit
- This model relies on you using your professional judgment for client care
- FT status nurses will be guaranteed 70 visits biweekly, allowing for the peace of mind of a guaranteed income
Responsibilities
- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
- When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community

Requirements
- Current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm for community care and commitment to evidence-informed practice and excellence in care
- A current CPR certificate is required
- A valid driver’s license and vehicle (travel required)
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire
- A willingness to attain required Saint Elizabeth certifications
- Availability to work days, evenings, nights and every other weekend
- This is a physically demanding position with lifting up to 55 pounds and routine movement
Return of Service eligibility
- Not have been employed as a nurse in Ontario in the six months prior to being hired
- Hold a Certificate of Registration in good standing from the College of Nurses of Ontario
- Begin their 2-year commitment between April 1, 2025 and March 31, 2026
- Commit to two-years of employment at a full-time schedule offered by the employer
- Not have previously participated in CCPN
- Not simultaneously receive funds from the Ministry of Health for the Tuition Support Program for Nurses
- Not be hired through the Nursing Graduate Guarantee program
