Qualifications

  • Current and valid state RN license is mandatory.
  • At least 1 year of recent acute care experience in an institutional setting.
  • BLS certification with hands-on CPR training required.
  • A valid driver's license and auto insurance is necessary.
  • Experience in home health is preferred.

Responsibilities

  • Conduct thorough assessments of patients and families for home care needs.
  • Provide physical assessments and document history of illnesses.
  • Develop and update personalized care plans with patient participation.
  • Identify and address discharge planning needs before patient discharge.
  • Offer counseling to patients and families for health needs.

Skills

Current and valid state RN license
Minimum of 1 year of recent acute care experience
Basic Life Support (BLS) certification
Valid driver's license and auto insurance

Education

Associate's or Bachelor’s degree in Nursing

Pay range (depending on experience): $50-$60 per hour
