The Registered Nurse (RN) is part of a team that provides direct care, education and support to older adults living in a long-term care home (LTCH); transitioning into or between healthcare and/or long-term care settings within the geographical boundaries of the South East Local Health Integration Network (SELHIN). Clients include those who are demonstrating responsive behaviours associated with various types of dementia, delirium, mental health, addictions, or other neurological conditions.

The RN is the lead contact for the Mobile Response Team, oversees and reviews initial intake and assessment, prioritizing referrals, crisis intervention, risk management and urgent behavioural support planning. The RN anticipates and recognizes subtle changes, probes to assess further, identifies relevant factors, understands significance and manages appropriately.

The RN develops plans of care to achieve identified client goals when overall care needs are highly complex and outcomes are unpredictable. The RN assumes the role of leader within the interprofessional team, providing clinical leadership, assessing changes, re-establishing priorities and determining the need for additional resources. In addition to LTCH staff, the RN will liaise with other community agencies such as Seniors Mental Health Outreach, PRC, CCAC, other practitioners, and key partners to coordinate care for clients. The RN may also contribute to program development and participate in the research activities of the program. The RN’s work is conducted in a manner that is consistent with the Mission, Vision and Values of Providence Care Centre.

Qualifications and Experience
  • Current unrestricted registration with the College of Nurses of Ontario
  • RN Diploma (BScN preferred)
  • Satisfactory criminal-record and vulnerable-sector screens
  • Minimum three years of related experience in providing support to older adults with responsive behaviours related to dementia, mental illness, addictions, or other neurological conditions
  • Training in P.I.E.C.E.S.TM, Gentle Persuasive Approach, U-First!, and Montessori Training is an asset
  • Canadian Certification in Gerontology and/or Mental Health preferred
  • Current CPR/First Aid certification
  • Experience with quality-improvement processes an asset
  • Extensive experience with various care settings working with clients who demonstrate responsive behaviours in conjunction with complex care needs
  • Prior experience working in collaborative practice settings
  • Strong written and oral communication skills
  • Ability to work effectively with others in an interprofessional care team
Role Responsibilities
  • Oversees initial intake in a timely manner with referred individuals, family members, and primary caregivers as appropriate
  • Assesses risk, type of service requirement (RN, RPN, and/or PSW) and assigns appropriate level service
  • Provides urgent behavioural support planning for referred individuals and their caregivers
  • In collaboration with other Behavioural Support Services and key partners, identify and refer to appropriate crisis resources for high-risk individuals
  • Supports clients in the development and implementation of a Collaborative Care Plan tailored to the client’s individual needs in order to improve quality of life and assist in the management of responsive behaviours
  • Provides, in consultation with LTC home team and MRT RPN and PSW ongoing review and reassessment of Collaborative Care Plans as required
  • Provides timely and accurate documentation including assessments, Collaborative Care plans, and progress notes as required and in accordance to the College of Nurses of Ontario
