The Registered Nurse (RN) is part of a team that provides direct care, education and support to older adults living in a long-term care home (LTCH); transitioning into or between healthcare and/or long-term care settings within the geographical boundaries of the South East Local Health Integration Network (SELHIN). Clients include those who are demonstrating responsive behaviours associated with various types of dementia, delirium, mental health, addictions, or other neurological conditions.
The RN is the lead contact for the Mobile Response Team, oversees and reviews initial intake and assessment, prioritizing referrals, crisis intervention, risk management and urgent behavioural support planning. The RN anticipates and recognizes subtle changes, probes to assess further, identifies relevant factors, understands significance and manages appropriately.
The RN develops plans of care to achieve identified client goals when overall care needs are highly complex and outcomes are unpredictable. The RN assumes the role of leader within the interprofessional team, providing clinical leadership, assessing changes, re-establishing priorities and determining the need for additional resources. In addition to LTCH staff, the RN will liaise with other community agencies such as Seniors Mental Health Outreach, PRC, CCAC, other practitioners, and key partners to coordinate care for clients. The RN may also contribute to program development and participate in the research activities of the program. The RN’s work is conducted in a manner that is consistent with the Mission, Vision and Values of Providence Care Centre.
