A healthcare organization in Sioux Lookout is seeking a Registered Nurse to provide enhanced support for diabetes education and care. This role requires collaboration with interdisciplinary teams and involves responsibilities such as medication management, lifestyle modification, and foot care. The ideal candidate should be registered with the College of Nurses of Ontario, possess a CDE designation, and have strong communication and clinical skills. Under the general direction of the Manager of MHAP, RAAM, ACT, SLDP, CCDC and Telemedicine, the Registered Nurse provides enhanced support to patients registered to the Centre for Complex Diabetes Care (CCDC). The CCDC RN plans, develops, implements and evaluates diabetes education services, diabetes monitoring, foot and wound care. The CCDC RN will work collaboratively with all members of the interdisciplinary health care team at Sioux Lookout Meno Ya Win Health Centre (SLMHC) and partner agencies to support and enhance the services of the CCDC, and ensure the provision of safe, wholistic, patient centered care to our clients.
$40.24 – $57.68; in accordance with ONA Collective Agreement
