Position Overview

The Care Company is seeking a skilled and dedicated Physiotherapist (PT) with strong home and community care experience to join our growing team. This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require in‑person home visits with vulnerable community members.

Key Responsibilities
  • Provide follow‑up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression
  • Develop, implement, and adjust individualized treatment plans in alignment with client goals
  • Educate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment use
  • Identify and address fall risks, environmental barriers, and safety issues in the home
  • Collaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated care
  • Complete documentation and charting accurately and in a timely manner according to community‑care standards
  • Maintain ongoing communication with the office regarding availability, scheduling updates, and urgent visit capacity

Requirements
Scheduling

Flexible full‑time and part‑time (Monday‑Friday), occasional Saturday/Sunday availability based on client needs.

Client Locations

Clients are located across Toronto East, including but not limited to:

  • Pickering, Ajax, Whitby, Oshawa, Bowmanville and select areas of Durham region
  • Kawartha Lake
  • Peterborough

Ability to independently manage scheduling, respond promptly to referrals, and coordinate visit times directly with clients.

Travel

Travel may be required between client homes.

Qualification and Experience Requirements

Home and community care experience is a MUST. Applicants without community experience will not be considered.

  • Minimum 2 years of physiotherapy experience in home and community care, preferably with HCCSS/OH@H clients
  • Registered and in good standing with the College of Physiotherapists of Ontario
  • Demonstrated experience in mobility assessments, gait training, falls management, equipment recommendations, and post‑discharge rehab
  • Comfortable working independently in the community and making sound clinical decisions
  • Strong communication and documentation skills
  • Ability to travel throughout Toronto and the GTA for client visits
  • Valid driver’s license and reliable transportation OR ability to navigate the GTA efficiently via transit
