Overview

The New Vision Family Health Team (NVFHT) is seeking a full-time (1.0 FTE), permanent Physician Assistant.

Located in the Kitchener-Waterloo region, the New Vision Family Health Team delivers comprehensive, team-based primary care through a collaborative interdisciplinary approach. Our team includes 16 Family Physicians and approximately 40 clinical and administrative staff, working together to provide high-quality, patient-centered care. We offer a professional, supportive, and collegial work environment within a spacious facility.

Position Summary

As a Physician Assistant, you will provide direct patient care within your scope of practice and under medical directives. You will conduct comprehensive and focused assessments, order and interpret diagnostic tests, participate in differential diagnosis, and develop and implement individualized care plans. You will promote preventative care and self-management strategies and advocate for the health and well-being of patients and their families. You will primarily be working with the practices of Dr. Rose Noble and Dr. Chase Reaume.

Main Responsibilities
  • Perform comprehensive and symptom-focused health assessments, considering physical, psychological, socio-cultural and environmental factors.
  • Formulate medical diagnoses in collaboration with supervising physicians.
  • Order and interpret diagnostic and laboratory tests in accordance with approved medical directives.
  • Share the responsibility to review and manage the electronic medical record (EMR) inbox, including lab and imaging results, specialist consult notes, patient portal messages and prescription renewal requests, ensuring timely follow-up and escalation as appropriate.
  • Complete and manage medical forms, including insurance, disability, school and workplace forms, ensuring accuracy, timeliness and compliance with clinical standards.
  • Develop and implement care and treatment plans in partnership with patients and their families.
  • Support patients with chronic disease management using pharmacologic, counselling and lifestyle strategies.
  • Document all assessments, interventions and outcomes in the EMR in accordance with NVFHT standards.
  • Collaborate effectively with the interprofessional team to ensure integrated and coordinated care.
  • Maintain appropriate confidentiality and adhere to all applicable regulations and policies.
Qualifications and Experience
  • Completion of an accredited Physician Assistant program.
  • Certification with the Canadian Certification of Physician Assistants (CCPA).
  • Active Registration with the College of Physicians and Surgeons of Ontario (CPSO).
  • Professional liability insurance.
  • Criminal Record Check with Vulnerable Sector Screening.
  • Health screening.
  • MMR, Varicella, Tdap, Hep B.
  • Exceptional communication, interpersonal and leadership skills.
  • Strong critical thinking and problem-solving ability.
  • Ability to work independently as well as collaboratively within an interprofessional team.
  • Primary care clinical experience is an asset that will be reflected in starting salary, however new grads are welcome to apply.
  • Experience with Telus PS Suite EMR is an asset.
