A leading home health care provider is seeking an experienced Physical Therapist to join their team for home health visits. This role involves assessing clients, developing care plans, and providing education in the clients' homes. Candidates must have a Colorado PT license and at least one year of PT experience. BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for an opportunity performing home health visits for our Fort Collins Visits team. This office works with adult and geriatric clients on a per visit basis in territories throughout Larimer and Weld Counties.
Territory: Larimer County (eastern), Weld County (western)
Schedule: Part Time, Full Time
Per Diem Weekend / Weekend SOC Physical Therapist position available, but candidate must have Medicare Home Health (and OASIS) experience.
One year prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
