Job summary

A community health service in Durham Region, Ontario, is seeking a Registered Nurse 3 to join the HKPR ACT team. Job Details

The Ontario Shores seeks a Registered Nurse 3 for the Haliburton‑Kawartha Peterborough Regional Assertive Community Treatment Team (HKPR ACT), which has offices in Peterborough and Lindsay, Ontario. Reporting to the Clinical Manager, the successful candidate will have sound knowledge of clinical issues in mental health and addiction.

In addition to mental health and physical assessments, responsibilities include: participating as a mental health professional on a multidisciplinary team; providing assertive community treatment to clients with severe and persistent mental illness; providing client‑centred treatment rehabilitation and support to referred clients; reviewing referrals in consultation with the team to determine eligibility for the program; developing treatment/rehabilitation goals and plans with clients, families, support network and multidisciplinary team; providing clinical case management services (psychosocial interventions, environmental adaptations, delivering medication, monitoring medication compliance, assisting and supporting clients in vivo with activities of daily living, harm reduction related to substance use, vocational rehabilitation, social relationships and use of leisure time); assessing for and recognizing signs of deterioration and intervening to prevent further hospitalization; providing emergency crisis intervention services to help the client and/or others.

Requirements
  • A current certificate of competence from the College of Nurses of Ontario, BSc N Preferred.
  • Current CPR - Health Care Provider (HCP).
  • Demonstrated knowledge of Psychosocial Rehabilitation and Recovery.
  • 3-5 years psychiatric experience in a mental health facility or service is desirable.
  • 3-5 years community‑based mental health experience.
  • Demonstrated knowledge of the College Standards of Practice and commitment to maintaining reflective practice standards as described by the College.
  • Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA.
  • An in‑depth knowledge and experience of the challenges involved in community reintegration of short and longer stay psychiatric inpatients with mental illness.
  • Ability to work effectively as part of a multidisciplinary team.
  • Demonstrated advanced knowledge of psychotropic medication.
  • Thorough knowledge and ability to administer mental status examinations as applied in mental health settings.
  • Demonstrated competence in individual and group programming skills.
  • The candidate will have demonstrated excellent interpersonal and effective oral/written communication skills.
  • Ability to work with clients of diverse ethno‑racial and cultural backgrounds; proficiency in a second language would be an asset.
  • Registered with the RNAO in good standing is preferred.
  • Ability to perform the physical demands of the position.
  • Variable hours may be required due to patient demand.
