Thenurse will have skills, training and experience, relating to wound management,vascular assessment and dressing application to provide and maintain a highstandard of nursing care for patients.Provide nursing assistance to the GPs, and other members of primaryhealthcare team, whilst providing supervision and working closely with a woundclinic Healthcare Assistant.

Theduties will (with training where necessary) include running a practice based woundmanagement, vascular assessment and dressings clinic, and providing a vaccinationservice to our housebound and care home patients. The nurse will undertake independentconsultations with patients relating to specific symptoms within the post holdersscope of practice; assessing, examining, investigating, diagnosing (withsupport where needed), treating, and following up patients as required.

Thenurse will use clinical decision making within their scope of practice and beresponsible for leading the wound clinic and housebound vaccination service forthe PCN Practices. They will ensure thedelivery of evidence-based care in relation to tissue viability including legulcer management.

Main duties of the job

Duties and responsibilities

Assist and perform routine tasks related to patientcare as directed.

Such tasks may include (list not exclusive):

  • Managementof patients with chronic disease relating to wound prevention and management,including diabetes and vascular conditions.
  • Providea holistic approach to wound management and implement wound care in line withcurrent evidence based guidelines.
  • Makeand receive referrals for patients with compromised tissue viability, fromnurses, general practitioners, consultants and other health professionals.
  • Provideadvice on the most appropriate wound dressing products.
  • Assessfor and advise on compression bandaging and hosiery.
  • Tobe responsible for the follow up of the patients under their care.
  • Provideadvice on management of slow healing wounds.
  • Practicewith autonomy and accountability and make decisions within the scope of ownprofessional practice area of clinical competence.
  • Promotea supportive and collaborative team-working environment.
  • Providesupervision and support to Healthcare Assistants.
  • Toidentify future service requirements to the Line Manager. Administrationadult vaccinations to our housebound and care home population.
  • Assistpatients to identify their health needs, incorporating the use of care plans.

Job responsibilities

The nurse will play animportant role in the PCN, utilising the appropriate skills, training andexperience to undertake independent consultations with patients relating tospecific symptoms; assessing, examining, investigating, diagnosing, treating,and following up patients as required.You will use clinical decision making within your scope of practice andensure the delivery of evidence-based care in relation to wound management andtissue viability.

This role will work closelywith GPs and practice teams to manage a caseload of patients, acting as acentral point of contact to ensure appropriate support is made available tothem and their carers; supporting them to understand and manage their conditionand ensuring their changing needs are addressed.
