Join our Nurse Practitioner team at the Lower Columbia Community Health Center in downtown Trail, BC. As a Nurse Practitioner, you will provide comprehensive, patient-centered primary care in a culturally safe environment. You will collaborate with an interdisciplinary team, support unattached patients and those referred through the Health Connect Registry (HCR), and engage with community agencies to address broader health needs.

Beyond direct care, you will contribute to local healthcare policies and participate in peer and self-review processes to enhance quality and outcomes. Key Responsibilities
  • Diagnosis and Treatment : Assess and diagnose undifferentiated diseases and conditions, provide treatment, and prescribe medications within scope and regulatory standards.
  • Care Management : Monitor ongoing care, order diagnostics, interpret results, and develop tailored treatment plans.
  • Patient Communication : Establish priorities, provide follow-up care, and communicate with patients and families about diagnoses and plans.
  • Collaboration and Consultation : Work with physicians and other professionals; may admit and discharge patients per policies.
  • Prescribing Medication : Prescribe medications within legal guidelines to ensure safe use.
  • Leadership and Supervision : Lead and guide other nursing and healthcare personnel; provide education and evaluations.
  • Research and Innovation : Contribute to research and initiatives impacting care and policies.
  • Health Promotion : Lead health promotion activities and screenings for at-risk populations.
