Overview

As a member of the Interdisciplinary Team, the Hospice Lead Social Worker works under the supervision of the Hospice Supervisor in the provision of supportive services to help meet patient/family social, emotional and economic needs in relation to the impact of a terminal illness. He/She supervises and/or provides social service activity as it relates to terminally ill patients and their families, including treatment planning and consultation with other staff members. Under the direction of the Home Health Supervisor, the Lead Medical Social Worker will help the organization provide a more comprehensive home health service by providing supportive services to help meet patient/family, social, emotional, and economic needs. The Lead SW provides oversite and guidance to the BSWs on staff per COPs and regulatory compliance.

Essential Job Requirements
  • Education: Masters Degree from a School of Social Work, accredited by the Council on Social Work Education.
  • Experience: Work experience in health related field for at least two (2) years.
  • Required skills: Ability to work independently and within a multidisciplinary team. Valid driver’s license required.
