Qualifications

• Current College of Nurses of Ontario registration in good standing.
• A valid driver’s license and vehicle for regular travel.
• Current immunization record including 2 step TB test.

Responsibilities

• Develop and maintain effective Care Plans with respect to Community Health Services.
• Provide exceptional nursing services both in-person and virtually.
• Communicate with patients and interdisciplinary care team members.

Current CPR Certification

Education

Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
