Salary: $51,901.07 - $59,325.32 per annum*
Summary
The Family Support Worker (FSW), Children’s Services is a full-time (37.5 hours per week), 12-month contract role based on-site, in-person in Geraldton, ON.
Working within a Recovery Framework, the FSW supports children and youth (birth to 17 years of age) living with mental health challenges and/or developmental disabilities, and their families/guardians and communities across the District of Thunder Bay. The FSW works as an integral part of a clinical team, and collaboratively with Children’s Counsellors, and is responsible to provide mental health and developmental supports, including but not limited to: targeted prevention (e.g., education, facilitation of psycho-social educational groups, presentations, etc.), case management and service coordination including referrals to specialized services, crisis response and family support. The FSW reports directly to the assigned Clinical Services Manager.
Responsibilities
- Facilitate the removal of barriers to care and connect clients to appropriate services via the Access Network and liaising with physicians, community agencies and care providers, ensuring wrapping around services to meet holistic client needs.
- Individually and/or collaboratively develop Personal Plans of Care with clients/families based on person-centered planning, self-determination and choice; provide services aligned with these plans.
- Provide education, information, referral and case management.
- Develop and/or facilitate psycho-educational and/or support groups for children, youth and families/guardians.
- Facilitate educational workshops and presentations.
- Ensure protection and rights of children and families/guardians are addressed in accordance with applicable legislation, Standards of Practice and NOSP policies and procedures.
- Maintain a client caseload and document all client engagement, case management activities and progress in the electronic client clinical record.
- Maintain confidentiality and work in accordance with the ethical guidelines of the Ontario College of Social Workers & Social Service Workers.
- Participate in clinical supervision and performance enhancement processes; develop personal goals for ongoing learning and self-evaluation.
- Assist, precept and provide support for colleagues, students and others to enhance clinical practice.
Qualifications/Requirements
- College Diploma in a field related to community and social services and/or human services; university degree in Human Service Field preferred.
- Eligibility for regulated membership in a regulated profession preferred.
- Three years’ experience in a mental health and/or developmental services agency providing interventions and/or support for children, youth and families preferred.
- Ability to support families and develop sustainable, professional relationships.
- Experience in goal setting and plans of care based on person-centered care planning, self-determination and choice.
- Experience with one-on-one supportive counselling, skill-building and group facilitation; development of psycho-educational groups, workshops and presentations.
- Knowledge of the Ontario child protection system and relevant legislation.
- Commitment to diversity, equity, and inclusion; anti-oppressive/anti-racist service delivery; understanding of Indigenous and BIPOC communities and related privacy and PHIPA practices.
- Independent and interdisciplinary teamwork abilities; strong written and oral communication; working knowledge of Microsoft Office.
- ASIST considered an asset.
- Understanding of scope of practice and regulatory standards.

- A Vulnerable Sector police check prior to commencement; positive checks reviewed for suitability to duties.
