About The Role

Provide a fulfilling day-away experience for individuals with varying degrees of dementia, memory loss, physical ailments (e.g. stroke), cognitive dissonance, and provide respite for caregivers. Ensure the needs (physical, social, cognitive, creative) of these individuals are addressed, and enhance each ones’ quality of life based on past and current interests.

Job Duties And Responsibilities

Assessment / Programme Development:

  • Evaluate needs of clients and families
  • Develop monthly event and activity calendars for caregivers
  • Develop, review, implement and evaluate activities for each client
  • Arrange for nursing assessment when necessary
  • Arrange transportation schedule
  • Assess in compliance with building protocols, dietary requirements, health and safety, WHMIS, Lambton County Policy and Procedure Manual, client charts, and day book routine

Programme Implementation

  • Supervise and assist clients during meal time and snack time
  • Set up and clean up meals
  • Promote, encourage and assist clients in socialization, activities of daily living, and communication
  • Ensure safe practices and procedures in carrying out all duties

Administration

  • Document as required - i.e. flow sheets, attendance, and client information
  • Requisition supplies for programme, and activities
  • Conduct ongoing audits
  • Liaise with Community/Other departments on site
  • Keep knowledge base current regarding aging and community supports
  • Maintain ongoing communication with clients’ families
  • Attend monthly staff meeting and check e-mail and voice mail daily

Assessments

  • Conduct new resident and quarterly recreational assessments, occupational therapy assessments and referrals; physiotherapy assessments and communication requests for referral, W/C and equipment referrals and assessment; Logo transfer assessments; referrals for speech/swallow assessments; and dietary needs assessments

Transportation

  • Drive and maintain vans
  • Develop efficient routes for transporting clients
  • Provide safe and efficient transportation of clients
  • Contact families/caregivers for programme cancellations due to inclement weather

General

  • Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed
  • Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis

What We Are Looking For

Minimum Formal Education

  • Personal Support Worker Diploma
  • Activation Certificate or Gerontology Diploma
  • Class F Driver’s Licence
  • Food Handler’s Certificate
  • CPR and First Aid Certificates are an asset

Experience

  • Three to four years of experience working with seniors and assisting with activities of daily living
  • Communication techniques, conflict resolution and knowledge, understanding, and experience with different types of dementia

A valid Ontario driver’s license and use of a vehicle.
