OVERVIEW

A growing, community-based organization providing shelter and outreach support for homeless youth in Peel Region. Recognized for its commitment to creating a safe environment and providing access to services that foster independence and growth.

MISSION

The Complex Case Coordinator offers care coordination to vulnerable and chronically homeless youth through assessment, planning, facilitation, care coordination, evaluation, and advocacy. The role empowers youth, leveraging their strengths to improve their quality of life by facilitating access to necessary supports and reducing homelessness risk.

The Pod Group partners with this organization to place a dedicated, resourceful individual in the role of Complex Case Coordinator.

PRIORITIES

  • Develop a system of care tailored to the unique needs of complex youth.
  • Manage case plans focusing on obstacles to housing success and promoting stability and achievement.
  • Monitor youth progress and adjust goals as needed through regular follow-ups.
  • Conduct weekly in-person check-ins to ensure goal progress.
  • Collaborate with staff to secure affordable housing using a Housing First for Youth model and ensure wraparound services are provided.
  • Support youth during viewings and meetings, advocating for their needs.
  • Teach life skills to help youth maintain housing, emphasizing tenancy skills.
  • Utilize assessments and care planning tools to coordinate services and advocate for youth needs.
  • Maintain positive relationships with landlords and community agencies for referrals and support.
  • Develop partnerships with health providers to ensure access to healthcare and other supports.
  • Maintain confidential files and complete documentation.
  • Perform other duties as assigned.

TALENTS & EXPERTISE

  • Bachelor’s Degree in Social Services or related field, or equivalent education and experience, with relevant certification (e.g., RSW, RSSW, RP, CYCP).
  • 2-3 years of case management experience.
  • Knowledge of housing first, trauma-informed care, and harm reduction.
  • Experience supporting youth facing chronic homelessness, mental health, and substance use challenges.
  • Ability to work rotating shifts, including weekends and holidays.
  • Valid Ontario Driver’s License, access to a vehicle, and minimum $2,000,000 liability insurance.
  • Valid CPI/UMAB and First Aid/CPR certifications.
  • Satisfactory Vulnerable Sector Police Check.
  • Flexibility to work at Brampton and/or Mississauga locations.
  • Must be fully vaccinated against COVID-19, as per legislation.
