Monday From : 16:00 To 22:30
Tuesday From : 13:00 To 17:00
Wednesday From : 16:00 To 22:30
Thursday From : 13:00 To 17:00
Friday From : 13:00 To 17:00

Hours may vary based on business needs.

The Care Coordinator is responsible for fostering a strong relationship between the clinic and its customers, including patients, staff, funders, and referral sources. This is achieved by efficiently providing administrative support and coordinating clinic activity with friendly, enthusiastic, and professional service with all patients, team members, and external customers.

The Care Coordinator handles patient intake, data entry, answering phones, outbound calls, receiving faxes and emails, invoicing, and payments.

Primary Responsibilities

  • Answer telephone calls, greet patients and visitors, and provide excellent customer service
  • Coordinate detailed and accurate intake of patient information during visits and treatment
  • Assist patients with paperwork to ensure accuracy
  • Schedule appointments and maintain patient records
  • Collect payments at the time of service
  • Track funding approval for all funding streams
  • Complete data entry and billing tasks
  • Manage courier packages and process emails, mail, and faxes
  • Provide administrative support to the Care Coordinator Manager and Clinic Director

Qualifications & Core Competencies

  • High School Diploma, G.E.D., or equivalent
  • Excellent customer service and telephone etiquette
  • Strong organizational skills and ability to prioritize and multitask
  • Excellent communication (verbal/written) and interpersonal skills
  • Strong computer and data entry skills
  • Flexible and adaptable to change
  • Experience in a Physiotherapy clinic is an asset
