Overview

Home Instead is looking for a dynamic and caring LPN Case Manager to join our team in Leduc. This is a permanent position with a Monday to Wednesday schedule that may start part-time and move to full-time. An active LPN registration is required. The Case Manager LPN will work with families at the beginning of their care journey, support Care Givers, conduct training, and hold family meetings. This role involves a consultative approach to determine each client’s needs and create a tailored service plan. It is a fast-paced, team-oriented position that emphasizes high-quality client care and support for our caregivers.

Responsibilities
  • Perform care consultations with potential clients and family members.
  • Introduce clients and Care Givers and conduct quality assurance visits with existing clients.
  • Utilize a consultative sales approach to identify client needs and develop tailored service plans.
  • Continuously evaluate care plans through ongoing communications and visits to ensure client satisfaction, retention, and opportunities to increase service hours.
  • Provide leadership by teaching and training unregulated staff, including classroom and in-home training.
  • Coordinate with families to explain care options and guide decision-making.
  • Maintain professional communication and act in a service-oriented team environment.
Qualifications
  • Current LPN registration is required.
  • Valid driver’s license with access to a vehicle.
  • Excellent written and verbal English communication skills.
  • 5+ years of experience in healthcare, with direct experience in senior/home care.
  • Experience in sales and customer service is preferred.
  • Supervisory experience is an asset.

This role is different from a clinic or care facility setting and involves visiting clients across the city.
