A regional health authority in Manitoba is looking for a Case Coordinator to manage client needs through effective health service delivery. This role requires independence, collaboration within a multidisciplinary team, and communication skills. Ideal candidates will have at least two years of relevant experience, an accredited degree, and a commitment to serving diverse communities.
Reporting to the Manager Health Services, the Case Coordinator will operate with a high degree of independence as a member of the multidisciplinary team. The incumbent is responsible for providing a collaborative, client‑driven process that assesses plans, implements, coordinates, monitors and evaluates the options and services required to meet the client’s health and human service needs through the effective and efficient use of resources within Home Care, Southern Health‑Santé Sud and the community.
The incumbent exercises the appropriate level of initiative and independent judgment in determining work priorities, work methods to be employed and action to be taken on unusual matters. The position functions in a manner that is consistent with the mission, vision and values; and the policies of Southern Health‑Santé Sud.
