As a Care Coordinator (Full-Time, Start date: January 5, 2026Location: Sudbury), you will assess and determine patient care needs andeligibility, provide access and referrals to community services, and engagewith patients, caregivers and other health care practitioners.

What will you do?

• In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

What must you have?

• Membership,in good standing, with the applicable regulatory body:

• OntarioCollege of Social Workers and Social Service Workers
• Recentexperience in community health or a related field
• Knowledgeof the health care delivery system and community resources
• Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
• Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
• Establishedability to accurately complete required documentation, reports and forms
• Avalid driver’s license and access to a reliable vehicle
• Advancedoral and written proficiency in English is essential

What would give you the edge?

• Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics

Salary: Hourly wage of $41.29 to $51.13
