As a Bilingual Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners.

What will you do?

Under the general direction of the Manager, Home and Community Care, the Care Coordinator is responsible for managing a caseload of diverse patients with various health concerns. The Care Coordinator will assess patients' care needs via phone, develop, implement, and update care plans based on health and psychosocial needs, and accommodate patient and caregiver wishes where possible. They will determine eligibility for Ontario Health atHome Champlain-approved services and refer patients and caregivers to community resources.

This role involves phone-based assessments and developing individual care plans using a multidisciplinary team approach. It is an in-office, non-visiting role, prioritizing referrals based on complexity, urgency, and needs, following Ontario Health atHome Champlain guidelines.

Primary Responsibilities:
  • Complete comprehensive patient assessments using validated tools, including clinical, physical, psychosocial, and caregiver needs
  • Assess understanding and learning needs related to diagnosis, treatment, resources, and coping
  • Assist in setting appropriate goals with patients and caregivers
  • Develop individualized care and service plans reflecting patient priorities and outcomes
  • Coordinate delivery of home and community care services
  • Manage vendor and resource utilization for medical equipment and supplies
  • Facilitate communication among healthcare team members to maximize outcomes
  • Implement and authorize care plans through contracted providers
  • Provide regular reports to physicians, service providers, patients, and caregivers
  • Document case information per Ontario Health atHome Champlain policies
Additional Details:

Team/Department: Intake

Hours of work: Rotating shifts (8 or 12 hours) between 8 am and 8 pm, 7 days/week, totaling 70 hours biweekly (12-hour shifts until June 7, 2025)

FTE: 1.0

Length of Temporary Assignment: December 5, 2025

Starting Salary: $44.480 to $47.517

Qualifications:
  • Registered Nurse (BScN or diploma) with the College of Nurses of Ontario; or
  • Degree/diploma in Physiotherapy with registration in Ontario; or
  • Degree/diploma in Occupational Therapy with registration in Ontario; or
  • Degree in Social Work with registration in Ontario; or
  • Degree in Speech Therapy with registration in Ontario; or
  • Degree in Foods and Nutrition or equivalent with registration in Ontario.
  • Related professional experience (community-based preferred), typically over two years
Preferred Skills:
  • Experience working with diverse patient groups and managing sensitive conversations
  • Ability to work independently and as part of a team
  • Strong communication, facilitation, diplomacy, and relationship-building skills
  • Proficiency with computer software and Microsoft Office
  • Valid driver’s license and access to a vehicle
  • Fluency in English and French
  • Recent Vulnerable Sector Check
