Department Description
  • One of the following: Bachelor of Social Work, Bachelor of Arts (Social Sciences), Bachelor of Science in Nursing, Bachelor of Psychiatric Nursing or Addiction Counseling Degree
  • Current registration in corresponding professional association.
  • Previous experience in the Mental Health and Addiction Services is an asset.
  • A valid driver’s license.

Job Description: Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement

Position Responsibilities
  • Screening, assessment, and coordination of all initial service requests (written, telephone, walk-in, psychiatry and court ordered referrals.) to MH/AS and determining appropriate service according to client need.
  • Responding promptly and effectively to initial requests for information and/or services, and planning other appropriate intervention or referral services for requests that are outside the mandate of MH/AS.
  • To prioritize and assign requests for service to the most appropriate program/professional in a timely manner. This may include gathering necessary data (i.e. previous assessment reports) before assigning to the program area.
  • Facilitate crisis response services and/or other short term clinical services to clients while waiting for a scheduled appointment with a MH/AS worker.
  • In cooperation with the program teams, assess complex cases and coordinate an appropriate response using an inter-disciplinary team approach.
  • In cooperation with the program teams, assist in development of the centralized intake program. This includes collecting data to monitor quality program indicators and identifying required policy or program changes.
  • Compiling and maintaining a list of community agencies and resources, inventory of MH/AS, recommended reading lists and fact sheets that can be distributed to clients.
  • Providing ongoing consultation within the agency and to other agencies and organizations to ensure continuity within the service systems.
  • Gathering, maintaining, and tabulating intake data and statistics utilizing the Provincial Mental Health and Addiction Information System (MHAIS), and the Centralized Intake Process
  • Maintaining files and records and ensuring confidentiality of all information received.
  • Behaving courteously and positively toward patients, clients, residents, visitors, and fellow employees.
