A healthcare staffing company is seeking a qualified Nurse to provide comprehensive patient-centered care in a home setting. The ideal candidate should hold a Bachelor of Science in Nursing or GNM and have at least 2 years of relevant experience. Job description

Summary: This role involves providing comprehensive patient-centered care to individuals with complex medical needs in a home setting. The primary responsibility is to ensure the safety, well‑being and optimal health outcomes of patients through skilled clinical assessment, ongoing monitoring and proactive intervention. The position requires a high level of clinical expertise in airway management, tracheostomy care, medication administration and emergency response along with strong communication and documentation skills. The ideal candidate will work collaboratively with physicians, therapists and families to deliver coordinated compassionate care while maintaining strict adherence to clinical protocols and regulatory standards. This role is essential in supporting patients' independence, quality of life and continuity of care within the home environment.

Responsibilities
  • Conduct thorough initial and ongoing patient health assessments to inform care strategies.
  • Develop, implement and update individualized care plans in collaboration with physicians and the multidisciplinary team.
  • Maintain a patent airway at all times, including regular suctioning using sterile technique and ensuring proper humidification of inhaled air.
  • Keep emergency equipment readily available at the bedside, including spare tracheostomy tubes, obturators, suction machines, oxygen sources and ambu bags.
  • Perform daily tracheostomy stoma care including cleaning, tie/holder changes and inner cannula replacement per protocol.
  • Monitor for signs of infection, skin breakdown or granulation tissue at the stoma site, and assess for tube obstruction or displacement.
  • Administer prescribed medications via oral, injectable or PEG‑tube routes and monitor for adverse effects and interactions.
  • Perform wound care dressing changes and pressure sore management as needed.
  • Conduct routine vital signs monitoring including blood pressure, temperature, heart rate, respiratory rate, oxygen saturation and blood glucose, and promptly report abnormalities.
  • Deliver enteral feeding via PEG or NG tubes and monitor nutritional and hydration status.
  • Educate patients and families on disease management, medication schedules and lifestyle adjustments.
  • Train family caregivers in essential care techniques and home‑based interventions.
  • Maintain consistent communication with healthcare providers including doctors, therapists and case managers to coordinate care and follow‑up.
  • Respond promptly to early warning signs of medical emergencies and initiate CPR or first aid as needed.
  • Maintain accurate, timely and detailed medical documentation of patient conditions, interventions and responses.
  • Provide empathetic patient‑centered care and emotional support to patients and their families to foster trust and comfort.
Requirements
