Job summary

Upperline Health is seeking a Practice Success Partner to guide partner practices through the Value Based Care program enrollment, ensuring understanding of processes, documentation, and technology. This role involves onboarding, data tracking, partner engagement, collaboration with care teams, and quality improvement to drive enrollment success across partners.

Qualifications

• Onboarding partner practices through Value Based Care enrollment.
• Experience deploying programs with a technology component.
• Strong communication and training delivery skills.
• Ability to work independently and as part of a team.
• Excellent organizational and time management skills.
• Proficiency in computer systems and data entry.

Responsibilities

• Guide enrollment, educate partners on process, resources, and required forms.
• Analyze enrollment data to identify trends and improvement opportunities.
• Build strong relationships with partner clinics and provide ongoing support.
• Collaborate with care teams and stakeholders to ensure program success.
• Contribute to process improvements and communicate opportunities to leadership.
