Job summary

Upperline Health is seeking a Practice Success Partner to guide partner clinics through Value Based Care enrollment, ensuring smooth processes, documentation, and technology use. You will educate clinic leads and doctors, track performance, and drive engagement across the program.

Travel up to 50% is required to partner practices along the East Coast to Midwest; remote applicants from AL, FL, GA, IL, IN, KY, MD, MI, NV, PA, SC, TN, TX, DC are encouraged.

Qualifications

• Requires healthcare experience; knowledge of value-based care preferred.
• Experience deploying programs with a technology component.
• Strong communication and interpersonal skills for cross-team engagement.
• Ability to deliver training effectively and engage clinic staff through storytelling.
• Good organizational and time management skills; data familiarity.
• Capable of working independently and as part of a team.

Responsibilities

• Onboarding partner practices through the Value Based Care enrollment process and educating clinic leads/doctors.
• Analyze enrollment data to identify trends and mitigation opportunities; coordinate with clinics.
• Build relationships with partner clinics and provide ongoing patient support during participation.
• Collaborate with care teams and stakeholders to ensure program success.
• Contribute to quality improvement initiatives and communicate opportunities to leadership.
