Job summary

Upperline Health, the nation’s largest provider focused on lower extremity, wound and vascular care, seeks a Practice Success Partner to guide partner practices through Value Based Care enrollment. You'll educate clinic leads and doctors on processes, documentation, and technology to ensure milestones are met.

The role faces travel up to 50% across the East Coast to Midwest; remote applicants residing in AL, FL, GA, IL, IN, KY, MD, MI, NV, PA, SC, TN, TX, and DC are welcome.

Qualifications

• High school diploma or equivalent; associate's or bachelor's degree preferred.
• Previous experience in healthcare is required; Knowledge of value-based care programs is preferred.
• Previous experience deploying programs with a technology component
• Excellent communication and interpersonal skills, with the ability to build rapport quickly across all areas of clinic staff engage through story-telling
• Demonstrated ability to provide training in an effective manner
• Demonstrated ability navigating conflict
• Strong organizational and time management skills.
• Proficiency in computer systems and data entry.
• Ability to work independently and as part of a team.

Responsibilities

• Onboarding partner practices through the Value Based Care enrollment process and educating teams on required documentation and technology.
• Analyze enrollment data, identify trends, and coordinate improvement plans with clinics.
• Build and maintain relationships with partner clinics to support ongoing engagement.
• Collaborate with care teams and stakeholders to ensure program success.
• Support quality improvements and communicate opportunities to leadership.
