Job summary

CINQCARE in Atlanta seeks a Practice Success Partner to own relationships with assigned practices under value-based contracts. You will analyze contract performance data to steer improvement and serve as the primary point of contact for the practices.

You will lead workflow adoption, coordinate with the Quality, Risk Adjustment, and Network teams, and guide leaders through performance gaps while protecting boundaries and escalating as needed.

Qualifications

• Five or more years of experience in a practice-facing, account management, or provider relationship role within value-based care, managed care, or provider organization.
• Demonstrated ability to read a contract performance report and convert it into a prioritized action plan.
• Ability to own a leadership-level relationship with practice leadership and physicians.
• Working knowledge of value-based care contract mechanics: quality measures, risk adjustment, transitions of care, and medical expense drivers.
• Proficiency with Excel and standard office software; ability to learn payer portals, analytics platforms, and internal tracking tools.

Responsibilities

• Translate contract performance data into a specific, practice-level action agenda and rationale.
• Maintain a focused set of priority actions per practice, reviewed with leadership on a fixed cadence.
• Own the relationship with assigned practices and be the primary contact, escalating when needed.
• Drive adoption of workflows to improve contract performance and coach integration into operations.
