Upperline Health is seeking a Practice Success Partner to guide partner practices through the Value Based Care enrollment, ensuring understanding of processes, documentation, and technology. This role requires traveling up to 50% to partner practices along the East Coast and offers remote options for eligible residents.

• Healthcare experience and knowledge of value-based care is preferred.

Responsibilities

• Onboarding partner practices through the Value Based Care enrollment process.
• Analyze enrollment data to identify trends and improve outcomes.
• Support partner clinics with root-cause analysis and mitigation plans.
• Build strong relationships with partner clinics to foster engagement.
• Collaborate with care teams to ensure program success and quality improvements.
