Job summary

Upperline Health is seeking a Practice Success Partner to guide partner practices through the Value Based Care program enrollment, ensuring understanding of processes, documentation, and technology. You will lead onboarding sessions with clinic leads and doctors, educate partners about program expectations, available resources and guide practices through resources to ensure successful milestones.

Qualifications

• High school diploma or equivalent; associate's or bachelor’s degree preferred.
• Experience in healthcare is required; knowledge of value-based care programs is preferred.
• Previous experience deploying programs with a technology component.
• Excellent communication and interpersonal skills, with the ability to build rapport quickly across all areas of clinic staff.
• Demonstrated ability to provide training in an effective manner.
• Demonstrated ability navigating conflict; strong organizational and time management skills.
• Proficiency in computer systems and data entry.
• Ability to work independently and as part of a team.

Responsibilities

• Onboarding: Guide partner practices through the Value Based Care program enrollment process, ensuring understanding of enrollment process, required documentation, applicable technology (hardware and apps), forms and compliant processes.
• Data Tracking and Performance Mitigation: Analyze enrollment data to identify trends and opportunities for improvement. Partner with partner clinics to understand root cause of issues and develop mitigation plans.
• Partner Engagement: Build strong relationships with partner clinics to foster trust and engagement in the program. Provide ongoing support and education to patients throughout their participation.
• Collaboration: Collaborate with care teams, providers, and other stakeholders to ensure program success.
• Quality Improvement: Contribute to process improvement initiatives to enhance partner enrollment efficiency and effectiveness. Communicate improvement opportunities to internal leadership.
