Upperline Health is seeking a Practice Success Partner to guide partner practices through Value Based Care enrollment, ensuring understanding of processes and required technology. You will conduct onboarding sessions and educate clinic leads and doctors, guiding practices to milestones.
Responsibilities include analyzing enrollment data, identifying trends, and working with clinics to develop mitigation plans. Build strong relationships and collaborate with care teams to drive program success.

This will require ~50% travel across Florida and Southern Georgia.

Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
