Qualifications

Responsibilities

• Conduct face-to-face home visits for ML TSS/D-SNP/FIDE and non-MLTSS members to assess medical needs.
• Develop care plans and authorize services in a cost-effective manner within benefits.
• Coordinate care with member, PCP, and care team; document accurately in EHR; mentor new hires.
