Qualifications

• 3+ years clinical practice experience in hospital or similar setting.
• Active and Unrestricted RN license in NJ.
• Willing and able to travel up to 75% in Bergen & surrounding counties, NJ; mileage reimbursed.

Responsibilities

• Conduct face-to-face home visits for MLTS/DSNP members using assessment tools.
• Develop and implement care plans; coordinate with PCP and care team.
• Advocate for members to ensure safe discharge and appropriate services.
• Document accurately in the electronic health record in a timely manner.
• Mentor new hires once proficient.
