Qualifications

• Must reside in Hudson and/or surrounding counties in New Jersey.
• 3+ years clinical practice experience (e.g., hospital, home health, ambulatory care).
• Active and Unrestricted RN license in NJ.
• Willingness to travel up to 75% of the time in Hudson and surrounding counties; mileage reimbursed.
• Successful completion of the NJ Choice certification with exam score 80%+ and state training modules.

Responsibilities

• Conduct face-to-face visits in members' homes to assess needs for MLTSS/D-SNP and non-MLTSS members.
• Develop care plans and authorize services within the MLTSS/FIDE benefit.
• Coordinate care with PCP and care team to ensure safe discharge and appropriate services.
• Document accurately in the member's electronic health record.
• Mentor new hires once proficient in the role.
