Job summary

CVS Health Corporation in New Jersey seeks a Case Manager Registered Nurse for a field/remote role with up to 75% travel in Union and surrounding counties. Monday–Friday, 8am–5pm EST, home visits, and collaboration with the care team to assess needs and coordinate services under MLTSS and D-SNP programs.

You will develop care plans, document in the EHR, support discharge planning, and mentor new hires after proficiency.

Qualifications

• Must reside in Union County.
• Active NJ RN license.
• Willing to travel 50-75% within NJ.
• NJ Choice certification required.
• 3+ years clinical practice.
• Experience in case management or discharge planning.
• Medicaid/Medicare experience is a plus.
• Proficient with MS Office 365.

Responsibilities

• Conduct face-to-face home visits using assessment tools for MLTSS/D-SNP members.
• Develop care plans and coordinate services with the care team.
• Advocate for safe discharge with appropriate services.
