Qualifications

• 3+ years clinical practice experience in hospital or home health/ambulatory care.
• Active and Unrestricted RN license in NJ
• Willingness to travel 50-75% in Hudson and surrounding counties, NJ
• NJ Choice certification required as contingency of employment

Responsibilities

• Conduct face-to-face visits in members' homes using assessment tools for MLTSS/DSNP/FIDE and non-MLTSS members.
• Coordinate care planning with care team to obtain needed services while controlling costs.
• Schedule and attend interdisciplinary meetings and advocate for safe discharge with appropriate services.
• Document accurately in the member's electronic health record in a timely manner.
• Collaborate with member/authorized representative, PCP, and other care team participants.
