Qualifications

• 3+ years clinical practice in hospital or alternative care settings.
• Active and Unrestricted RN license in NJ.
• Willingness to travel up to 75% in Bergen & surrounding counties.

Responsibilities

• Conduct face-to-face home visits for MLTSS/D-SNP members.
• Develop and implement care plans with the care team.
• Coordinate with PCPs and other providers for services.
• Document accurately in the electronic health record and ensure HIPAA compliance.
• Mentor new hires after attaining proficiency.
