Job summary

CVS Health is seeking a Field/Remote Case Manager RN to support members in Bergen County and surrounding areas. The role requires coordination of care planning, documentation in the electronic health record, and collaboration with PCPs and care teams.

The ideal candidate holds a NJ RN license, at least 3 years of clinical experience, and can travel up to 75%. NJ Choice certification is a contingency for employment and mileage is reimbursed.

Qualifications

• 3+ years clinical practice experience (hospital or similar)
• Active and Unrestricted NJ RN license
• Willing and able to travel up to 75% in Bergen County and surrounding counties
• NJ Choice certification requirement as contingency of employment

Responsibilities

• Develop, implement, and promote health service strategies and programs for members.
• Coordinate care planning with providers and interdisciplinary teams.
• Document in the EHR accurately and timely.
• Advocate for safe discharge and appropriate services.
• Mentor new hires after achieving proficiency.
