Qualifications

• 3+ years clinical practice experience
• Active NJ RN license
• Must reside in Bergen County and be willing to travel up to 75% in Bergen & surrounding counties
• NJ Choice certification completion required as a contingency of employment

Responsibilities

• Conduct face-to-face home visits for MLTSS and D-SNP/FIDE members.
• Develop and implement care plans with the care team.
• Collaborate with PCP and other providers to coordinate services.
• Document accurately in the member's electronic health record.
• Mentor new hires after proficiency is attained.
